We provides 24 hrs. /7 days a week homecare services to individuals of any age who may require assistance in their homes and communities to assist in the maintenance and retention of their independence and well-being. The services are responsive to the distinct needs of the individual and community and are delivered by a qualified staff.
American Family Home Health Agency will begin services by carefully assessing the patients’ health condition to determine his/her unique requirements prior to implementing any services. We will identify health needs of individuals and families which will include the physical, medical, social and emotional aspects of the assessed individuals’ condition.
After a comprehensive health services is assessed, services appropriate to the needs of the individual and family are planned, coordinated, and made available by the agency.
Skilled Private Duty Nursing
Private duty nursing services consists of four or more hours of continuous skilled nursing services provided in the home to eligible clients with complex medical needs. Services are provided by licensed professionals and assistants through visits or hourly care. Skilled nursing may be provided in a package of care or a specific disease management package of skilled care/services.
Certified Nursing Assistant
Services is provided through visits or hourly care to clients with complex care needs and is under the direct supervision of a registered nurse. Services included personal care activities of daily living and additional duties as delegated by the registered nurse according to the plan of care.
Services is provided through visits or hourly care to individuals with to stable clients with non-medical needs, and is under the direct or indirect supervision of a registered nurse. Services Include personal care activities of daily living according to the plan of care.
Services is provided through visits or hourly care and includes homemaking, housekeeping and other activities inside and outside the home which support client independence and do not include personal care services.
Non-medical transportation to social activities and/or adult day care center.
Is support on a short-term basis for the person who receives the waiver to offer caregivers a break.
